
About the Role
The Activity Coordinator assesses, plans, implements, and evaluates programs for patient activities that are multi-faceted, meet the patient’s functional needs, and reflect the interests of each patient.
DUTIES AND RESPONSIBILITIES:
Plans, schedules, implements, and evaluates a program of individual and/or group activities based on the patient’s needs.
Coordinates and participates in patient activities designed to promote social interaction, reality orientation, and intellectual stimulation.
Completes the activity assessment and care plan following the gathering of information to reflect the individual patient’s functional level, problems, needs, and interests.
Documents activity-related information on the patient’s chart. Review staff’s chart entries for completeness.
Assists with escorting patients to and from activities.
Maintains sufficient activity supplies.
Develops monthly calendar of planned activities.
Develops and maintains a pleasant and pleasing environment in the Activities Room.
Develops and reviews continuous quality improvement.
EDUCATION
Must have a high school diploma and at least a year of experience working with the frail elderly or with adults with special physical or mental needs.
Must provide at least three reference letters or the names of individuals with whom a reference interview can be conducted, including at least one former employer. The individuals providing reference information shall have knowledge of the applicant activity coordinator's background and qualifications.
First Aid and CPR certification required.
PHYSICAL REQUIREMENTS
Must be at least 18 years of age
Must present a written medical statement, completed within the prior 12 months by a physician, nurse practitioner, or physician’s assistant, certifying that the employee has no illness or health condition that would pose a health risk to others and that the employee can perform the duties assigned in the job.
