About the Role

LCAS will coordinate and monitor the array of services and supports identified in each service user’s person-centered plan. These interventions are strength-based and focus on the promotion of recovery, symptom reduction, increased coping skills, and the achievement of the highest level of social functioning.

TASKS AND RESPONSIBILITIES
• Provide service to SACOT and SAIOP Persons Served
• Conduct clinical assessments
• Conduct individual, family, and group outpatient therapy sessions
• Provide group leadership to include education and skills-related groups
• Orient new Persons Served to Southeastern Healthcare of NC, Inc., CRI
• Manage and monitor the caseload of active treatment Persons Served;
• Monitor Persons Served progress, safety, and medications;
• Coordinate/conduct drug screens and Breathalyzer tests;
• Develop person-centered plans for Persons Served
• Oversee the discharge planning process
• Facilitate relationships between Southeastern Healthcare of NC, Inc., CRI, service user families/legal guardians, and various social services and community resources, such as food stamps and housing assistance, healthcare, job training and placement, and substance abuse support groups
• Provide group leadership to include education and skills-related groups
• Participate in clinical supervision
• Participate on mini/mega teams for case consultation
• Conduct weekly aftercare groups
• Conduct weekly family groups
• Document all services provided on progress notes
• Complete and submit monthly reports
• Perform other clinical duties as needed in the program

KNOWLEDGE, SKILL AND ABILITY
• Ability to support the company's mission, vision, and values and abide by applicable standards of conduct, policies, and procedures.
• Ability to interact effectively with people from diverse backgrounds.
• Ability to communicate effectively, verbally and in writing.
• Ability to problem-solve issues.
• Proficient in Microsoft Office Suite applications (i.e., Word, Excel, PowerPoint, Outlook specifically),
and database uses.
• Must be honest, dependable, and able to meet deadlines.
• Must be organized and able to handle multiple tasks.
• Self-motivated, ability to manage time and tasks independently.
• General working knowledge of the human services delivery system in North Carolina
• Valid driver’s license, auto insurance, and reliable transportation
• Employee supervisory experience preferred
• Experience with Electronic Medical Records (EMR) preferred

EDUCATION AND FORMAL TRAINING
• Graduate degree in Social Work or a related field required; Master’s degree preferred
• Valid licensure to provide mental/behavioral healthcare in the state of North Carolina
• Minimum 2 years experience working with individuals with mental and/or substance abuse and/or behavioral health issues that include writing person-centered plans, conducting effective group therapy and providing mental health/substance abuse diagnoses
