Community Living and Support Services (CLS) are home- and community-based services funded through the North Carolina Innovations Waiver for adults with intellectual and developmental disabilities. This innovative program helps eligible individuals gain new skills and supports them in activities so they can live successfully at home and become more involved in the community.  

 

CLS can help develop independent living skills, enabling individuals with I/DD or TBI to stay in their homes or live with their families. CLS also offers technical assistance to family members or those living with the individual to help them maintain the skills they have learned.

 

Assistance with personal care includes skills like self-care, hygiene, and clothing. In-home skill building emphasizes independence and daily tasks. Our CLS program helps individuals learn or improve skills such as building relationships, living independently, and working toward 

• Boosting their self-sufficiency

• Making their own choices

• Meeting personal goals outlined in their PCP or ISP

• Engaging fully in their community

 

The goal is to maximize independence, increase self-determination, and improve opportunities for full community integration appropriate for their level of independence.

 

 

Eligibility

Eligibility for Community Living and Support Services depends on a disability diagnosis and an individual assessment of the person’s abilities and needs. Services are provided based on a personalized service plan.

 

Insurance Information

Community Living and Support Services are funded through the North Carolina Innovations Waiver for adults with intellectual and developmental disabilities. Monarch accepts Medicaid and various private insurances. Southeastern Healthcare of North Carolina currently provides CLS services for Alliance Health Plan.
