 Senior Services

Through our non-profit agency, Southeastern Wake Adult Day Center, we offer services to help alleviate the strain of in-home and in-community care of older adults and adults with disabilities who have a desire to remain in their own home.  Through our Adult Day, Overnight Respite, Home Care, and other therapeutic services, we assist seniors and disabled adults with vital activities of daily living, health monitoring and social interaction.

Adult Day Health provides the peer engagement, mental stimulation, and physical support that seniors and disabled adults need in their later years. Adult Day Health also provides reassurance so family members can rest easy knowing their loved one is in a safe, nurturing environment that caters to the concerns of every participant.

 Behavioral Health Services

Generally, behavioral health refers to mental wellbeing.  Behavioral health services address mental health disorders, substance use disorders, life stressors, personal crises, and stress-related physical symptoms. The behavioral health services and programs are designed to help with the diagnosis, prevention and treatment of these conditions.  Our teams of credentialed professionals work to develop person centered treatment plans and work closely with our clients to realize their goals and live their best lives.

Psychosocial Rehabilitation (PSR) helps adults diagnosed with chronic mental illness learn how to understand and manage their illness in order to improve daily functioning. We provide a diverse range of interpersonal and professional development activities aimed at increasing knowledge and skills as it relates to accomplishing individual goals and objectives.

Peer Support Services give individuals hope to support each person in setting and achieving person centered goals in their recovery. Peer support benefits individuals who are determined to have balanced, healthy life.  Our program is designed to enhance the quality of life and improve the general well-being of our clients.

Our Substance Use Recovery Programs provides the support that people need to end their dependence on substances and begin a process of change through which they improve their health and wellness, live self-directed lives, and strive to reach their full potential. Even people with severe and chronic substance use disorders can, with help, overcome their illness and regain health and social function.

Community Health Services

Southeastern Healthcare has a dynamic Community Health Worker (CHW) Program that has been providing needed outreach, support and referral services to community members in need.   From being frontline responders during the recent Coronavirus pandemic to helping to address health equity and the social determinants of health, CHWs are proving to be vital members of a broader healthcare provider network.
