
About the Role
The CNA/PCA will provide personal care and other supportive services to patients under the supervision of a registered nurse and in accordance with the Person-Centered Treatment Plan. The CNS/PCA will provide personal care and other supportive services to patients under the supervision of a registered nurse and in accordance with the Person-Centered Treatment Plan.
SPECIFIC DUTIES:
Provide all aspects of personal care and support services as directed and under the supervision of a registered nurse. This includes, but is not limited to:
Assisting patients with personal care including bathing, skin care, and mouth care.
Assisting patients with ambulation, including ambulation with a walker
Assisting patients in and out of bed
Assisting patients with meal preparation and eating
Assisting patients with using the bedpan, commode, or bathroom
Perform household services, which will facilitate the patient's self-care at home and are necessary to prevent/postpone hospitalization
Immediately report any changes in the patient's condition to the patient's case manager.
During or immediately after the visit, document all the care provided as outlined in the Certified Home Health Aide Plan of Care.
Ensure that safety measures are being followed. This includes, but is not limited to:
Ensuring there are no loose rugs in the patient's ambulation path
Keeping the side rails up on the bed (if appropriate)
Ensuring that the patient is secured in the wheelchair (if appropriate)
Complete the orientation program outlined by the organization
Participate in in-service education programs
Participate in the Interdisciplinary Treatment Meeting
HEALTH STATUS REQUIREMENTS:
TB screen
Hepatitis B vaccine
or signed waiver
MINIMUM COMPETENCIES/REQUIREMENTS:
Eligible to participate in a personal care assistance program.
Good verbal and written communication skills.
Ability to follow instructions in accordance with written policy.
Must provide at least three reference letters or the names of individuals with whom a reference interview can be conducted, including at least one former employer. The individuals providing reference information shall have knowledge of the applicant's health care coordinator's background and qualifications.
